Century Park Associates is proud to offer continuum-of-care campuses at many of our locations so that our residents don't have to worry about what happens when their needs change.
A continuum-of-care campus allows residents to easily move between different levels of care without having to search for a new care facility and leave their familiar community and friends behind.
Finding a retirement community that is a perfect fit is a lot of work. As we age and our health begins to change, the level of independence we are capable of maintaining, as well as the level of help and care we may need from others, can also change. These changes can be difficult, and no one wants to have to also be looking for a new community to call home during those stressful times.
Partnering with our affiliate company Life Care Centers of America, Century Park's continuum-of-care campuses provide multiple levels of care - from independent living to assisted living, as well as long- or short-term skilled nursing, all in one location. Skilled nursing facilities (sometimes called nursing homes or convalescent centers) provide housing and 24-hour skilled nursing services to those requiring post-acute care due to decreased ability to perform activities of daily living and offer rehabilitation services as well.
